New students of Trails can now register without going to our Centers. Adapting to the new normal, we have improved our Student Registration Process so parents can be in their homes when they register their child at Trails.
The new process involves the following steps:
INQUIRE WITH OUR FRONTDESK. Inquiry can be done through calling any of our mobile phone numbers, through sending a message in our Facebook page or emailing us at firstname.lastname@example.org. Our friendly Frontdesk Officers will ask some basic questions about the child and your contact information.
FILL OUT THE ONLINE STUDENT HISTORY FORM. After giving your email information to us, you will receive an access code to our Online Student History Form. Access the Online Form using Google Chrome and fill up the fields correctly and completely until you reach the end. Please make sure you upload the necessary documents e.g. referral letter from the doctor, etc. Click Finish when you reach the end of the form. We will then schedule a ZOOM meeting between the parents and our Program Director. A separate email invitation to the ZOOM Meeting will be sent to you.
INTERVIEW WITH THE PROGRAM DIRECTOR. Teacher Weng, our Program Director, will discuss the current options of service delivery and may ask for some clarifications based on the information you submitted in the Student History Form. Your preferred day, time and frequency of therapy will also be discussed.
MATCH WITH THERAPIST. Our Frontdesk Officer will contact you regarding the therapist who will be assigned to your child based on your discussions with our Program Director. Once you agree with the proposed schedule, we are ready to commence the evaluation and therapy sessions.
We will keep on improving our processes as we see fit.
